Are the exact locations for each Starlink installation already determined? If so, how many? If not, is there an estimated number per village?

Each village will have vastly different numbers. Some may be as low as 20, and some as high as>1K. Addresses have yet to be gathered.

Will AVI purchase and receive all the hardware? If so, where will it be staged, and will the Contractor be able to perform testing?

AVI will purchase and deliver the hardware to the appropriate community for staging.  We anticipate that the contractor will conduct testing during installation.

Will the vendor be responsible for ongoing maintenance and troubleshooting post-installation?

No, ongoing maintenance and troubleshooting beyond the installation are not part of the scope.

Can you confirm whether AVI and/or the customer will cover all Starlink account set-up fees, etc.?

All equipment will be included from AVI, Starlink does not require set-up fees.
